A puff-puff is a sensual massage that is used as a comical side event in many Dragon Quest games. It is completely unrelated to the story and plot of any game and appears only as a sort of practical joke. A puff-puff has a fair amount of sexual innuendo, which is what led to it being removed from the first 3 Dragon Quest games on the NES. All of the Dragon Quest games in the main series feature puff-puffs in some fashion.

A puff-puff is typically only available to a male character and is mentioned only by female non-playing characters. After the male character agrees to the puff-puff massage, the screen goes dark and the text *puff* *puff* typically appears in the dialog box. This may or may not be accompanied by various massage sound effects. The screen then ceases to be dark and often something humorous occurs or is spoken by the female character. The exact details of the massage are left unknown as part of the sexual humor of the event. However, it is generally believed to involve the rubbing of the woman's breasts on the male character in some fashion. This type of joke is consistent with other Japanese sexual humor.
Appearances
Dragon Quest
In the original version of Dragon Quest, a woman in the village of Kol near the open air baths will offer to give the hero a puff-puff. For the NES release, her dialog was changed to not reference puff-puff in any way. However, in the SNES fan translation and GBC version of the game, the puff-puff dialog and associated massage return.
Dragon Quest III
In Dragon Quest III a woman in the town of Ashalam offers the hero a puff-puff at night. In the NES translation of the game, this has been completely changed to be a fortune telling instead of a puff-puff massage.

Dragon Quest IV
In Dragon Quest IV, a male character can receive a puff-puff massage in Monbaraba at night during chapter 5.
